Instructions for CARES Final Report and FS-10-F

The CARES Final Report is available in the online portal. It is a separate survey from the application. The final report and FS-10-F are required in order for the final 10% of the award to be released.

Process:

  • Complete online final report
  • Complete FS-10-F using the form provided in the portal (there is a link to the FS forms at the top of the final report page)
  • Email draft FS-10-F to NYSLCARESAct@nysed.gov so that it can be reviewed before it is signed and mailed to the State Library
  • Once State Library staff have reviewed the final report and the draft FS-10-F and emailed any corrections, the final version can be sent to the State Library. Please send 3 copies with original signatures in blue ink.

Outputs/Outcomes:

Systems were recently sent a list of data outputs and/or outcomes that are required for IMLS reporting, and there may be other outputs/outcomes that were listed in your application. Please provide all of that information. There are questions in the final report that cover many of the outputs. For other outputs, and for outcomes, you may attach files as needed.

Numbers should be entered for procurement, content, consultations and presentations:

Procurement: Enter item type/name (ie, laptop, etc.), number purchased and number used. If a box containing multiple items was purchased, enter the number of items in the box. More than one item may be entered in each of the four categories.

Content: Enter name of online or print resource and enter quantity created.

Consultations: Enter consultation type (ie legal advice, etc.), total consultations and average per month.

Presentations: Enter presentation title, length of presentation, number of presentations and average attendees per session.

Final Expenses:

Information from the system’s application (amended application if that applies) has been pulled into the final report. It can be adjusted if necessary (for example, if quantities changed). Additional information must be entered for each relevant category.

FS-10-F

Use the information entered in the final expenses section to complete the FS-10-F. A template is provided in the “FS Forms” link and the system’s name, address, etc. has been pre-filled.
